We are offering a one-time sign-up opportunity for a Death Benefit for active members (who did not previously sign up) during the months of November and December 2023. Scan the QR code to get started!
(If you did not previously sign up for the Death Benefit, a one-time charge of $10 will be added to your dues billing).
You can also scan the QR code to update your beneficiaries. If you do not remember whether you signed up, or who you listed as your beneficiaries, we urge you to re-fill out the form.(There is no charge for updating your beneficiaries).
