Please contact the Health Benefits Department for the forms that you need to use these services. DRUG DIVISION MEMBERS DO NOT REQUIRE FORMS.

The member must sign the form to authorize services.

Advise your dentist that payment takes 4-6 weeks should you choose to use someone not on this list.
